CC (chief complaint): “My supervisor set up this appointment for me. I work in a large
architectural engineering firm and it's been all great. But recently, they've accelerated the
deadlines and it just puts a lot of pressure on me, and I just can't concentrate. I mean, everyone
else doesn't have a problem with it. But, I just can't seem to be able to do the same job they're
doing”
HPI: H.G. is a 58-year-old Caucasian male who presents to the clinic for concentration and
attention issues. The patient stated that he is an engineer who works in a large architectural
engineering firm. His job has been going on fine until his firm began accelerating their deadlines.
This has resulted in him losing concentration and attention. Due to this problem, the patient
states that he makes silly mistakes that could cost the company millions of dollars. The patient
has a history of attention and concentration issues back in school but it was never treated. He was
easily distracted when doing an assignment in the library, or studying for an exam. He also stated
that in his job, he hardly pays attention whenever the chief of his department delivers a lecture.
The patient has issues with staying organized; he easily forgets his shoes, socks, phone, and
jacket. He was gifted a calendar by his coworkers to assist him stay focused and organized. In
this calendar, he wrote down vital information and important events for him to easily remember.
However, he hardly looks at the calendar or keeps tracks of the important events. Additionally,
he hardly pays attention to his bills; he only pays when he has received threatening calls/letters
or he has to pay a penalty.
