Patient Information:
 Name: 62-Year-Old Male
 Height: 6'1" (185 cm)
 Reason for Encounter: Frequent urination – "I’m peeing all the time."
 Location: Outpatient clinic with x-ray, ECG, and laboratory capabilities
Reason for Encounter:
The patient presents with complaints of frequent urination, a common issue for older males. This
condition can often be attributed to various underlying causes, including benign prostatic
hyperplasia (BPH), overactive bladder (OAB), diabetes, or urinary tract infections (UTI). The
focus will be on identifying the primary cause of this symptom, followed by appropriate
treatment.
History of Present Illness (HPI):
 Chief Complaint: Frequent urination, occurring both during the day and night.
 Symptoms: Increased frequency of urination, urgency, possible nocturia (waking up during the
night to urinate), no significant pain or burning during urination.
 Onset: Symptoms began gradually over the past several months.
 Exacerbating Factors: Drinking large amounts of fluids, especially caffeinated beverages.
 Relieving Factors: Reducing fluid intake and urinating more frequently during the day.
 Associated Symptoms: No significant pain, fever, or hematuria (blood in the urine).

 When did you first notice the frequent urination?
 "I first noticed it about 3 months ago, but it has been getting worse over the past few weeks."
 How long has this problem been occurring?
 "It’s been happening for a few months now, maybe around 3 months."
 Have you experienced any changes in the frequency of urination over time?
 "Yes, it has definitely increased. At first, I would go to the bathroom a couple of times during the
day, but now it’s much more often, sometimes every hour or so."
 Is the frequent urination happening more during the day or at night?
 "It’s mostly at night. I wake up at least 3-4 times every night to go to the bathroom."
 Do you wake up during the night to urinate? If so, how many times per night?
 "Yes, I wake up at least 3 times every night."
 Are you experiencing any urgency to urinate, or do you feel the need to go suddenly?
 "Yes, sometimes I feel like I have to go urgently, and if I don’t go immediately, it becomes
uncomfortable."
 Is there any pain or burning sensation when you urinate?
 "No, there’s no pain or burning. It’s just the frequency."
 Do you ever feel like you haven’t fully emptied your bladder after urinating?
